Chocolate Fundraiser Agreement & Inventory Responsibility Form

Fundraiser Overview

The Shelton HS Cheer Booster is conducting a chocolate fundraiser to support All American registration and travel to London.

Each cheerleader will be issued 4-5 boxes of chocolate. Each box has a value of $80. The exact number of boxes issued to your Cheerleader will be tracked. Families may: Sell the chocolate and return funds, or return any unsold, unopened inventory by the designated deadline. Parent/Guardian Responsibility Agreement

By signing below, I acknowledge and agree to the following:

1. Inventory Tracking

   I understand that all chocolate boxes issued to my cheerleader will be tracked by the Shelton HS Cheer Booster.

2. Financial Responsibility

   I accept full responsibility for the total value of all chocolate boxes received by my cheerleader.

3. Payment or Return Requirement

   I agree to either:

   • Submit full payment ($80 per box), OR

   • Return any unsold, unopened chocolate by the established deadline

4. Unreturned Inventory

   I understand that any chocolate not returned or paid for will be considered sold, and I will be responsible for the full amount owed.

5. Billing Agreement

   I acknowledge that if payment is not received for distributed inventory, the Shelton HS Cheer Booster reserves the right to bill me for the outstanding balance.
